Forces with a current cause of concern
A cause of concern is the inspectorate's most serious inspection finding. It is a separate mechanism from the performance level a force is assigned under the Policing Performance System, and a force can carry one without being moved off Level One. 15 of 44 forces currently carry at least one open cause of concern, 26 in total. HMICFRS told the force in its September 2024 report to improve non-emergency call abandonment rates and incident attendance times; sufficient progress hasn't been made. Inspectors also found incidents being downgraded from priority without sufficient risk assessment or supervisory oversight, in many cases where the circumstances reported by the caller hadn't changed, with missed opportunities to safeguard vulnerable domestic abuse victims.

Clare's Law / Domestic Violence Disclosure SchemeAccelerated
HMICFRS found Suffolk Constabulary doesn't have the necessary processes, capability or capacity to properly operate the Domestic Violence Disclosure Scheme (Clare's Law). A backlog of over 500 outstanding 'right to ask' applications was identified, including one submitted in January 2025, preventing timely disclosures under the 'right to know' process. The constabulary wasn't fully aware of the risks this backlog posed. It was also creating an identifiable digital record of police contact on some victims' devices (leaving emails and texts), with no standardised risk process for cases where an offender might answer the victim's phone or overhear a call. This constitutes a report under section 54 of the Police Act 1996, requiring a response from the local policing body under section 55. Carried into the 2025-27 report as a cause of concern under safeguarding.

Safeguarding children and adults at risk of harm
The constabulary must make sure it identifies, assesses and mitigates risk to safeguard vulnerable people. It has no secondary review process for standard-risk domestic abuse cases, an area for improvement first raised in the 2023-25 inspection and not resolved. There is no process by which multiple calls trigger an escalation in risk level, and identified changes in risk level aren't consistently applied to police systems. In the year ending 31 January 2026 the constabulary recorded 616 domestic abuse cases as high risk, of which 238 were later downgraded to medium and 20 to standard, a downgrade rate of 41.9 percent, which the constabulary hasn't scrutinised to establish whether the downgrades were appropriate. Inspectors found cases inappropriately downgraded from high risk and then passed to non-accredited investigators. A constabulary audit in July 2025 showed only 42.0 percent of high-risk cases reviewed had a domestic abuse research document in place. Four recommendations issued with a three-month deadline.

Providing a safe and lawful custody environment
The constabulary doesn't have an effective and safe detainee handover process. Authorised professional practice states custody officers and staff should carry out shift-change handovers together and communicate information in person. Inspectors found the remote written handover process insufficient and that it increased risk to detainees and personnel. The constabulary responded by introducing a video recording of the outgoing officer reading out the written handover, which still doesn't allow in-person clarification or risk discussion. Inspectors saw instances during suite observations where risk information was missed during handover. The immediate risk hasn't been sufficiently mitigated and still remains. Two recommendations issued with a three-month deadline.

Policing Performance System levels
On 16 July 2026 HMICFRS assigned every police force in England and Wales a performance level for the first time. Levels run from Level One, the default position, to Level Four, special measures. They are decided three times a year by the Policing Performance Monitoring Group, chaired by His Majesty's Chief Inspector of Constabulary. The levels sit alongside PEEL rather than replacing it, so a force has both a set of PEEL grades and a level.

- Level 1 · Default
- The default position for all forces. No action or intervention is required from the performance system at this time.
- Level 2 · Enhanced support
- Enhanced support is available to prevent any deterioration in performance. Applies where a force has low-level, non-systemic issues, issues which are getting better but require oversight, or multiple minor issues.
- Level 3 · Targeted support and intervention
- Targeted support and intervention is provided and chief constables develop improvement plans. Applies where a force has a single, critical and lasting issue, widespread or systemic performance concerns, or fails to demonstrate meaningful improvements.
- Level 4 · Special measures
- Applies where performance failings are very serious and there is insufficient evidence or confidence that steps being taken to address them are having the required effect.

A fifth, on managing registered sex offenders, has been closed. Eight areas assessed, with no custody section for this force. Four Good, four Adequate, no Requires Improvement or Inadequate grade anywhere and no cause of concern. Four areas for improvement: use of force recording and disproportionality analysis, recording of neighbourhood policing abstractions, consistency of crime outcome type, and fraud training for investigators and call handlers. This entry replaces the 2023-25 cycle entry previously held for Cumbria. Relevant to VNS's evidence base: the constabulary recorded 34 of 35 antisocial behaviour personal crimes correctly in the quality service review, against 16 of 25 for Suffolk in the same cycle. Outcome 16, evidential difficulties where a suspect is identified and the victim does not support further action, accounted for 29.4 percent of outcomes assigned to victim-based crimes in the year ending 31 December 2025, higher than the typical range for forces in England and Wales, and inspectors said the constabulary needs to better understand why victims stop being involved. The correct outcome was applied in only 56 of 70 cases reviewed and victims' views were considered in 43 of 53. On use of force, the constabulary recorded 11,754 incidents in the year ending 31 March 2025 against an estimated 13,170, a shortfall of 1,416 and a deterioration on the previous year's estimated 627. People from a Black ethnic minority background were 8.5 times more likely than White people to be subject to use of force and 5.8 times more likely to be stopped and searched, and 24 percent of use of force records between April 2025 and February 2026 lacked self-defined ethnicity. Independent domestic violence advisers are co-located in the control room in line with the principles of Raneem's Law. Under the Domestic Violence Disclosure Scheme the constabulary made 1.4 right to know applications per 1,000 population, the second highest rate nationally, against 0.7 right to ask applications per 1,000, among the lowest nationally.

- Clare's Law / Domestic Violence Disclosure Scheme: HMICFRS found Suffolk Constabulary doesn't have the necessary processes, capability or capacity to properly operate the Domestic Violence Disclosure Scheme (Clare's Law). A backlog of over 500 outstanding 'right to ask' applications was identified, including one submitted in January 2025, preventing timely disclosures under the 'right to know' process. The constabulary wasn't fully aware of the risks this backlog posed. It was also creating an identifiable digital record of police contact on some victims' devices (leaving emails and texts), with no standardised risk process for cases where an offender might answer the victim's phone or overhear a call. This constitutes a report under section 54 of the Police Act 1996, requiring a response from the local policing body under section 55. Carried into the 2025-27 report as a cause of concern under safeguarding.
- Safeguarding children and adults at risk of harm: The constabulary must make sure it identifies, assesses and mitigates risk to safeguard vulnerable people. It has no secondary review process for standard-risk domestic abuse cases, an area for improvement first raised in the 2023-25 inspection and not resolved. There is no process by which multiple calls trigger an escalation in risk level, and identified changes in risk level aren't consistently applied to police systems. In the year ending 31 January 2026 the constabulary recorded 616 domestic abuse cases as high risk, of which 238 were later downgraded to medium and 20 to standard, a downgrade rate of 41.9 percent, which the constabulary hasn't scrutinised to establish whether the downgrades were appropriate. Inspectors found cases inappropriately downgraded from high risk and then passed to non-accredited investigators. A constabulary audit in July 2025 showed only 42.0 percent of high-risk cases reviewed had a domestic abuse research document in place. Four recommendations issued with a three-month deadline.
- Providing a safe and lawful custody environment: The constabulary doesn't have an effective and safe detainee handover process. Authorised professional practice states custody officers and staff should carry out shift-change handovers together and communicate information in person. Inspectors found the remote written handover process insufficient and that it increased risk to detainees and personnel. The constabulary responded by introducing a video recording of the outgoing officer reading out the written handover, which still doesn't allow in-person clarification or risk discussion. Inspectors saw instances during suite observations where risk information was missed during handover. The immediate risk hasn't been sufficiently mitigated and still remains. Two recommendations issued with a three-month deadline.
- Responding to the public: The constabulary was taking too long to answer emergency and non-emergency calls. In the year ending 31 March 2023 it answered only 76.6 percent of 999 calls within 10 seconds.Closed August 2026Closed by HM Inspector Roy Wilsher in the PEEL 2025-27 report following improvement. Directly relevant to VNS's subject matter: in the quality service review inspectors examined 25 antisocial behaviour personal cases and found 16 recorded correctly, with seven harassment crimes not recorded at all. The constabulary under-recorded use of force, recording 4,784 incidents in the year ending 31 March 2025 against an estimated 10,693, a gap of 5,909 and a deterioration on the previous year's estimated shortfall of 4,266. People from a Black ethnic minority background were 5.0 times more likely than White people to be stopped and searched and 4.9 times more likely to be subject to use of force, with disproportionality on both measures increased against the year ending 31 March 2024. MARAC referral rejection was 48.3 percent in the year ending 31 March 2026, up from 39.9 percent in 2025, and the constabulary hasn't fully reviewed those decisions. Inspectors found a domestic abuse allegation not resourced for six days under the C grade call process.
